If you have a concern regarding any aspect of your care, please contact 409-962-5733 during regular business hours (9:00 a.m. to 5:00 p.m., Monday through Friday, excluding holidays) and ask for Administration.  For all other hours, please contact the Nursing Supervisor by calling 409-962-5733.  If your complaint continues to be unresolved, you may also call the Department of State Health Services (DSHS) at 888-973-0022 or write to Health Facility Licensing and Compliance Complaints, Department of State Health Services (DSHS), 1100 West 49th Street, Austin, Texas 75756-3199.
