How to Enroll
To enroll in our Diabetes Self-Management Education (DSME) program or to make an appointment for personal nutritional counseling, contact your primary care physician or endocrinologist and have them fax a referral to Renaissance Hospital at 409.963.1069.

To help patients with diabetes manage their long-term health, we offer a comprehensive Diabetes Self-Management Education (DSME) program. Prior to attending Diabetes Education classes, each enrollee will participate in an individual assessment with our Registered Dietitian or Nurse Educator to ensure that participation in the program meets their unique needs. While individualized nutrition and meal planning are provided as part of the class curriculum, additional personal counseling with our Registered Dietitians is available with a physician referral. We also welcome all past and present Diabetes program participants to attend our quarterly support groups.
